Forms & Acknowledgements

In an effort to streamline player onboarding, MFPA has moved collection of all the paperwork required for your athlete to participate this season to a secure online solution. Using GoFormz, you can complete all of your forms in a platform, built on Microsoft Azure, which uses industry best security practices and is compliant with GDPR, SOC 2, ESIGN, and HIPAA regulations.

You will be completing all of the required paperwork other than the physical in ONE (1) form submission. So, please allow yourself plenty of time (estimated 20-30 minutes). You’ll want to have your medical information, insurance policy, and your player, ready to sign. This form can be completed on your desktop, tablet, or phone. Note, you will be required to draw your signature. So, it may be easier to complete on a touchscreen device, but it is still doable on your desktop computer.

There are 3 steps to completing your player’s onboarding with the team: Complete the Paperwork, Pay the Fee, and Turn in the Physical. The physical is the one form that will need to be handed in to coaches. We have also added a place to upload below to ensure we have a complete electronic file and in case of lost paper forms. You can download a blank form below or your doctor should have copies. All forms, player fee payment, and physical must be received prior to Iron Mohigan. If you are unable to pay your player fee in full by this time, please complete the Financial Aid Request form to request full or partial payment assistance or complete the contact form to get in touch with a Board member to set up a payment plan.
